Pharmacological action
Antipsychotic drug. The mechanism of the expressed antipsychotic action is connected with blockade of dopamine receptors in CNS. Results in moderate sedative effect. Besides the preparation renders antiemetic, analgesic effect , febrifugal, anticonvulsant and antihistaminic action.
Pharmakokinetics
Absorbtion
When administered orally - about 70 % of a dose it is absorbed from gastrointestinal tract.
Distribution
At injecting - 92 % of active substance bind with fibers of plasma.
Metabolism
Haloperidol is metabolized in liver.
Elimination
The preparation is eliminated from an organism with renal and bile excretion, is exposed to hepatic recycling. T1/2 makes 13-40 h.
Indications
* psychomotor agitation of various genesis (including at mania, dementia, oligophrenia , psychopathy, reactive and chronic schizophrenia, alcohol addiction);
* delirium and hallucinations of various origin (at reactive and chronic schizophrenia, paranoid states, acute psychosis);
* acute worried depression syndrome with agitation (the agitated depression);
* Huntington's chorea;
* agitation, aggression, behavioral disorder at elderly and children's age;
* logospasm;
* stubborn, resistant to therapy vomiting and hiccup;
* Gilles de la Tourette's syndrome.
Dosage
Dosage is adjusted individually depending on age, clinical picture and the previous response of the patient to other neuroleptics. At acute psychoses adults administer the preparation dose of
5-10 mg intravenously or intramuscular. If necessary the abovementioned dose can be repeated once or twice with 30-40-minutes interval until the desirable therapeutic effect manifest itself. The maximum daily dose makes 30-40 mg.
In case of acute alcoholic psychosis a dose of 5-10 mg of Haloperidol is injected, if necessary the preparation injection is repeated
At delirium alcoholicum accompanied with agitation, Haloperidol in a dose of 10-20 mg is injected intravenously; usually injecting speed makes 5-10 mg/min (not more than 5 mg/30sec).
For oral administration an average daily dose for adults makes 2.25-18 mg. If necessary the dose can be incremented until steady therapeutic effect is obtained, and then to reduce it gradually until lower maintaining dose is reached. If clinical improvement does not appear during 1 month it is not recommended to continue treatment.
Collateral actions
CNS-related: in the therapy beginning drowsiness can be observed; at longer application are possible akinesia, tremor, increase of a muscle tone and other symptoms of parkinsonism, and also developing of a delayed dyskinesia.
Contra-indications
* diseases of central nervous system accompanied with pyramidal or extrapyramidal disorders
* pregnancy;
* a lactation (chest feeding);
* depressions (except the agitated depressions).
Pregnancy and lactation
Haloperidol is counter-indicative for administration at pregnancy in view of possible negative effect on fetus development.
If treatment with Haloperidol is needed during lactation period the chest feeding must be stopped.
Special instructions
Alcohol is forbidden during the treatment. In case of simultaneous application of Haloperidol with indirect anticoagulants when Haloperidol administration is discontinued the anticoagulant dose must be corrected.
Drowsiness and other parkinsonism symptoms arising at Haloperidol administration, are stopped spontaneously when the preparation dose is reduced or when the treatment is discontinued temporarily. If delayed dyskinesia appears it is necessary to lower gradually a dose of Haloperidol and to prescribe another preparation.
Influence on capability to drive motor transport and to manipulate mechanisms
For patients who administer Haloperidol, it is necessary to abstain from potentially dangerous kinds of the activity requiring special attention and fast psychomotor reactions.
Overdose
Symptoms: muscles tension, general or local tremor, drowsiness, an arterial hypotension, sometimes - an arterial hypertention. In extremely hard cases - coma, respiratory depression, shock.
Treatment: the specific antidote does not exist. In case of oral administration if patient status does not preclude, it is possible to perform a stomach lavage then to prescribe an activated coal. At respiratory depression it is necessary to resort to artificial pulmonary ventilation. For blood circulation improvement plasma or albumin solution is injected intravenously, as well as noradrenaline ( at such cases to apply adrenaline is categorically forbidden!). In order to reduce intensity of extrapyramidal symptoms it is necessary to inject antiparkinson drugs, for example benztropine mesylate.
Drug interaction
If administered with other preparations Haloperidol boosts action of antihypertensive agents, opioids, barbiturates, antidepressant, ethanol and ethanol-bearing preparations. Simultaneous application of Haloperidol with indirect anticoagulants results in action degrading of the latters.
