Quetiapine is also more affinitive to histamine and a1- adrenoreceptors and less affinitive in relation to a2-adrenoretseptors.

Pharmacological action.

Antipsychotic drug (neuroleptic). It is more affinitive to receptors of serotonine (5-HT2), than to receptors of dopamine D1 and to D2 in the brain. Manifesting tropism to histamine and alfa1-adrenoreceptors, is less active towards alfa2-adrenoreceptors. No selective affinity to m-choline - and benzodiazepine receptors was found. Reduces activity of mesolimbic À10- dopaminergic neurons, in comparison with À9-nigrostriatic neurons, involved motor functions. Does not cause long lasting increase of lactation hormone concentration. Duration of link with 5-HT2-serotonine and D2-dopamine receptors makes less than 12 h.

Indications:

Acute and chronic psychoses (including at schizophrenia).

Contra-indications: Hypersensitivity, pregnancy, lactation period. Carefully. Arterial hypotension, epilepsy and epileptic seizures (in the anamnesis).

Collateral actions:

Nervous system related: drowsiness, dizziness, headache, anxiety, asthenia, hostility, agitation, insomnia, akathisia, tremor, spasms, depression, paresthesia, neuroleptic malignant syndrome (hyperthermia, muscle tension, changed mental status, lability of vegetative nervous system, increase of activity of creatine phosphokinase). In relation of systemic vascular resistance: orthostatic hypotension, tachycardia, prolongation of Q-T interval . Digestive system related: dryness of oral mucosa, nausea, vomiting, abdominal pains, a diarrhea or constipation, increase of activity of "hepatic" transaminase.

Respiratory system related: pharyngitis, rhinitis. Allergic reactions: skin rash, eosinophilia. Laboratory indicators: leukopenia, hypercholesteremia, hypertriglyceridemia, decrease in concentration of Ò4 (the first 4 weeks). Other: back pain, chest pain, low-grade fever, weight gain (mainly in the first weeks of treatment), myalgia, dryness of a skin, amblyopia.

Administration and dose:

Orally, twice a day, irrespective of food intake. A daily dose during the first 4 days of therapy - 50 mg (1 day), 100 mg (2 day), 200 mg (3 day), 300 mg (4 day). From the 4-th day, a dose should be titrated to be effective, within the limits from 300 up to 450 mg per day, if necessary – up to 750 mg per day. At nephritic and/or hepatic insufficiency and at elderly patients an initial dose - 25 mg per day, with the subsequent daily increase by 25-50 mg until effective dose is obtained.

Special instructions: During treatment it is essential to be careful at driving of motor transport means and at doing other potentially dangerous kinds of activity, requiring raised concentration of attention and speed of psychomotor reactions.

Interaction:

Does not cause induction of enzyme systems of the liver, involved into antipyrine metabolism. Inductors of microsomal liver systems (phenytoin, etc.), thioridazine raise extraction ratio of quetiapine, inhibitors of microsomal liver systems - reduce; at the same time simultaneous prescriptions of quetapine with energizers - imipramine (inhbitor of CYP2D6) or fluoxetine (inhibitor CYP3A4 and CYP2D6) does not substantially influence on its pharmacokinetics. Preparations, oppressing CNS, and ethanol raise risk of side-effects developing.
